Important Yale Student ID Card Information
NEW STUDENTS WERE REQUIRED TO SUBMIT A PHOTO FOR YALE UNIVERSITY ID CARD by May 31st (or as soon as your get a NetID if you are a late admit)
Yale University issues all students an official photo identification card to gain access to studios, buildings, transportation facilities, libraries and other services. Your Net ID and University ID card are your keys to the city at Yale.
Please be advised that Yale will use your preferred name* to generate your initial Yale email address and ID card. If you wish to change your preferred name or use your legal name for your primary Yale email address or your ID card, you must log in to the student information system after you receive your NETID and indicate this. Desired changes to the name displayed on your ID card must be made by August 1, before the cards are printed to avoid a charge for printing a new card.
In order to have your University ID card ready for you upon your arrival on campus, all incoming students must electronically submit a color PASSPORT PHOTO. If a Passport Photo is not available, you may submit a color snapshot showing a full front view of your head and shoulders. Please submit all photos no later than May 31st by going to:

Please note that this photo will also be used to create a School of Art face page directory which is used exclusively within the School.

Yale requires two items for the Health Clearance Requirement: an Immunization Record, and the completed Health Form/Physical Exam. (due to the COVID-19 pandemic if you cannot obtain a physical exam please provide a copy of your most recent one if it is from the last 1-2 years)
To upload Immunization Record, or to confirm receipt status of these items please login to [YALE MEDICAT PORTAL](https://yale.medicatconnect.com/default.aspx) using your Net ID
Make an appointment now with your doctor to complete the required Yale Immunization Record. Once completed, the form and your supporting documents must be uploaded directly to the Yale Medicat System (which will be available between June 20 - August 1).

Beginning on August 2nd the University will place a registration hold on students missing all required health documentation. Please be sure to complete the requirement ASAP to avoid any disruption to Fall semester course registration which will take place in the School of Art on Monday, August 31.

INTERNATIONAL STUDENTS IMPORTANT INFORMATION
Required documents for incoming student VISA APPLICATIONS are due by May 31st for the upcoming academic year. Submit the form linked below along with supplemental bank information as noted:
The Office of Academic Administration will receive and check over your International Student Financial Certification form and supporting documents. When all items are collected, they are forwarded to Yale’s Office of International Student Services (OISS) who handles the processing of I-20s and other immigration documents.
The general processing time for an I-20 is approximately two weeks from the date The School of Art receives your documentation. I-20s will be sent via Fex-Ed by the School office; please be sure the correct mailing address is updated with us.
